Referral Check List:
We are currently scheduling new clients 1-2 weeks out both via in-person and within a week for telehealth appointments. In order for us to get new client's scheduled as quickly as possible please provide the following information when you send over a referral:
- Full client name
- Phone number and email
- Insurance information with member id if available.
- Date of birth
